By December 2020 the project aims to recruit 20 new participants with 5 moving into employment and 8 moving onto Job Search or into education/training. The key worker will adopt a person centred approach to building relationships with the participants.
The key worker will be working with the participants to define their needs through a comprehensive assessment process which identifies outcomes based on their emotional, behavioural, personal and physical well-being, soft skills and functional skills. This then builds and develops an Individual Learning Plan which encapsulates personal tailored goals and desired outcomes, helping them to resolve any personal issues by offering advice and signposting to support agencies and in-house partner services that can offer the appropriate support to the participant.
Contact with participants will be on a weekly basis as a minimum (30 minute conversation/work on individual interventions) with at least 1 face to face meeting per month (1 hour). We will monitor our success by constantly assessing and monitoring where the participant is on their journey, setting new goals as existing goals are achieved and updating review logs at each meeting. We will work with participants for up to six months an average of 4 with ‘in work’ support provided.
